In this transcript the entries have been rearranged into numerical order though the layout follows the headings in the original document. All the information in the original has been transcribed, retaining all spellings as in the original. Surnames have been typed in block capitals to make it easier to search for this information and the order of the names is Christian name followed by surname where as in the original the surname is generally recorded first.
